Conscious Minds Mental Health aims to make access to mental health treatment accessible to all.

Conscious Minds Mental Health offers a fee schedule with cash pay options for a variety of services including medication management and therapy visits, preparing independent medical opinion reports, drafting accommodation letters, and completing disability and school forms.

Fee Schedule

Initial Evaluation: $150
This is your first visit, typically 45-60 min to obtain pertinent medical and psychiatric history and work together to create treatment goals.

Follow Up Appointment: $80
This is for all appointments after your initial evaluation. These appointments are typically 15-30 minutes and focus on making adjustments to medications and treatment plan. Depending on complexity of treatment & individual needs appointment follow-ups can range from weekly, monthly, bi-monthly, or quarterly.

Forms

$20-$50 per packet/entity
All documentation requests should be made during a follow-up appointment. This ensures that we have adequate time to discuss individual needs and goals. In most cases, care must be established and ongoing in order for forms to be completed accurately.

*Some of the most common requests include FMLA, disability, and school or work accommodations.

Superbills available by request for those seeking reimbursement for out-of-network services.

Cancellation Policy

If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay half the cost of the session.
